Monday - Friday 8:00 a.m. - 5:00 p.m.
Client Eligibility Criteria
1. Applicant must be verifiably homeless.
2. Applicant must be at least 18 years old.
3. Applicant must be pregnant, have children, or have a reunification plan within six months.
4. Applicant must have identifiable case management needs.

Program Description
The primary goal of Lincoln Action Program's Supportive Housing Program is to help families attain the life skills necessary to maintain permanent housing and prevent a return to homelessness. The program is designed to meet holistic family objectives by addressing barriers in areas such as parenting, child care, money management, medical, transportation, chemical dependency, education, mental health etc. The most important element of the Supportive Housing Program is the intensive case management service. All participants work closely with an involved case manager. With the help of their case manager each participant develops and signs a contract outlinig the goal areas the family will work on while in the program. Their continued participation in the program is dependent on their ability to follow through in these goal areas. A family can participate in the Supportive Housing Program for a maximum of two years (may be shorter depending on the needs of the family).
